  • Up to $25 back for successfully completing an authorized CPR or first aid course.
  • Birth Related Extras: Up to $50 back for a childbirth class, $50 back on the purchase of a breast pump, and $100 back for a lactation consultant

PREAPPROVAL/PRECERTIFICATION REQUIREMENTS
Prosthetics and Orthotics. Previously, prosthetics and orthotics with a purchase price of more than $100 required preapproval/precertification. Effective January 1, 2007, only those prosthetics and orthotics with a purchase price in excess of $500 will continue to require preapproval/precertication.

Durable Medical Equipment (DME). Previously, Durable Medical Equipment with a purchase price of more than $100 required preapproval/precertification. Effective January 1, 2007, only DME purchases of more than $500 will require preapproval/precertification.

Infusion Drugs. Effective January 1, 2007, the infusion drugs listed below require preapproval when administered in a professional provider’s office. Infusion therapy provided in an outpatient facility or home setting will continue to require preapproval/precertification.
